Peony is generally well tolerated for short-term use, though high-quality safety data are limited. The most common side effects when taken by mouth are gastrointestinal — abdominal bloating, loss of appetite, diarrhea, nausea, and general stomach discomfort.

In one clinical study, diarrhea occurred in about 5% of people taking a peony product at a moderately high dose for 24 weeks, compared to 1% on placebo.
