N'SANE is a 15-ingredient powder containing a mix of amino acids, vitamins, minerals, and herbal extracts. The active ingredients include thiamine (a B vitamin), L-histidine, caffeine anhydrous, L-tyrosine, vanadyl sulfate, yohimbe bark, creatine (as Kre-Alkalyn), L-arginine, and alpha-lipoic acid, along with evodiamine, phenylethylamine HCl, AKG, and glycine propionyl-L-carnitine (GlycoCarn) in proprietary blends.
Bitter orange extract (Advantra Z) rounds out the active list. The product also contains several inactive ingredients — silica, maltodextrin, natural and artificial flavors, citric acid, sodium citrate, sucralose, malic acid, and purple carrot — which serve as fillers, binders, and flavorings.
Does it work?
Strong evidence
Evidence of effectiveness is mixed and varies by ingredient. Thiamine is effective for thiamine deficiency and Wernicke-Korsakoff syndrome (a brain condition from severe deficiency); it is possibly effective for period pain but possibly ineffective as a mosquito repellent or for heart failure.
Caffeine is effective for neonatal apnea and postoperative headache, and likely effective for mental alertness and athletic performance. Creatine (Kre-Alkalyn) is possibly effective for muscle strength and athletic performance but possibly ineffective for Huntington disease and bone loss.
Alpha-lipoic acid is possibly effective for diabetic nerve damage, high cholesterol, and obesity. For several other active ingredients — L-histidine, L-tyrosine, vanadyl sulfate, yohimbe, L-arginine, and bitter orange — the evidence we hold is insufficient to rate their effectiveness for the conditions studied, or the ratings are not established in our data.
The proprietary blends and some individual ingredients (Evodiamine, Phenylethylamine HCl, AKG, GlycoCarn) also lack effectiveness data in our monographs.
How safe is it?
Well-documented data
Most ingredients are generally well tolerated in healthy adults, though some carry cautions. Thiamine is safe in normal amounts and needed during pregnancy and breastfeeding — use supplement doses only as your provider advises.
Caffeine is generally safe in moderate doses but can cause anxiety, insomnia, tremors, and nausea; high doses are risky, and pregnant or breastfeeding people should limit intake and check with their doctor. Beta-alanine (CarnoSyn) is well tolerated but commonly causes harmless tingling and skin flushing, and is best avoided in pregnancy and while breastfeeding.
Yohimbe carries significant safety concerns — it can cause serious heart and blood pressure effects, and supplement potency is unpredictable; it is unsafe in pregnancy and breastfeeding. Vanadyl sulfate is generally well tolerated below 1.8 mg daily but can cause stomach upset, diarrhea, and nausea at higher doses, with rare kidney damage at very high long-term doses; avoid in pregnancy and breastfeeding.
Bitter orange may raise heart rate and blood pressure, especially with caffeine or other stimulants, and is possibly unsafe in pregnancy. L-arginine and alpha-lipoic acid should be used under medical supervision and are best avoided in pregnancy and breastfeeding due to insufficient safety data.

Also check antihypertensive (blood pressure) drugs, anticoagulants and antiplatelets (blood thinners), antidiabetes drugs, seizure medications, tricyclic antidepressants, antipsychotics, stimulant drugs, thyroid hormone, liver-metabolized drugs, and any heart medications.

The most serious concern is yohimbe, which carries a Major-severity interaction with monoamine oxidase inhibitors (MAOIs) — combining them can cause dangerous additive effects. Bitter orange (Advantra Z) also has two Major interactions: with MAOIs (risk of hypertensive crisis due to tyramine and synephrine content) and with midazolam (sedative; bitter orange may raise blood levels and increase side effects).

Caffeine in this product interacts with ephedrine at Major severity, with a theoretical risk of serious cardiovascular adverse effects like heart attack or dangerously high blood pressure. Several Moderate interactions involve caffeine as well: it may reduce the effectiveness of sleep medications (pentobarbital, phenobarbital) and certain seizure drugs (carbamazepine), and it may increase levels of the antipsychotic clozapine or certain antibiotics (quinolones).
Bitter orange adds more Moderate interactions — it can inhibit the metabolism of many drugs handled by the liver (CYP3A4 substrates), increase heart rate and blood pressure when mixed with other stimulants, and theoretically worsen the effects of blood-sugar-lowering drugs.
Yohimbe carries six additional Moderate interactions: antihypertensive drugs (yohimbe may raise blood pressure and interfere with control), certain liver-metabolizing enzymes (CYP2D6 and CYP3A4), stimulant drugs, antipsychotics (phenothiazines), and tricyclic antidepressants. Other ingredients present Moderate risks with blood thinners and blood-sugar drugs (vanadyl sulfate and L-arginine), blood pressure medications (L-arginine), potassium-sparing diuretics (L-arginine), heart medications like sildenafil (L-arginine), cancer drugs and thyroid hormone (alpha-lipoic acid), and levodopa for Parkinson's disease (tyrosine).

Trimethoprim (Proloprim)
Trimethoprim might increase blood levels of thiamine.
In vitro, animal, and clinical research suggest that trimethoprim inhibits intestinal thiamine transporter ThTR-2, hepatic transporter OCT1, and renal transporters OCT2, MATE1, and MATE2, resulting in paradoxically increased thiamine plasma concentrations.
