Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Nattokinase is a single active ingredient—a protein enzyme extracted from natto, a traditional Japanese fermented soybean food. It works by breaking down fibrin, a protein that helps form blood clots, which is why it's been studied for cardiovascular conditions.
The product is delivered in a softgel capsule with inactive ingredients that include rice bran oil, gelatin, glycerin, and several others that serve as fillers and preservatives.
Does it work?
Not established
The evidence for nattokinase isn't established yet in our data. We hold insufficient reliable evidence to rate it for angina, atherosclerosis, cardiovascular disease, cancer, and chronic fatigue syndrome.
That doesn't mean it doesn't work—it means the clinical trials so far haven't been robust enough to confirm a benefit. Talk with your doctor if you're considering it for any of these conditions.
How safe is it?
Well-documented data
Nattokinase is generally well tolerated in the short term, but its blood-thinning effect means caution if you have a bleeding disorder or are at risk for bleeding. Rare cases of bleeding have been reported with nattokinase use, including a serious case of brain hemorrhage.
Additionally, some people are sensitive to natto proteins and can have allergic reactions, from mild throat swelling to anaphylaxis, usually within 2 hours of taking it. The safety data doesn't support use during pregnancy or while breastfeeding.
If you're pregnant or nursing, avoid this product and talk with your doctor or pharmacist about your options.
Meds to double-check
Moderate interaction found
Before you start, check if you take any blood thinners (anticoagulants or antiplatelets) such as warfarin, aspirin, clopidogrel, or dabigatran—nattokinase raises your bleeding risk with these and has caused serious bleeding in at least one patient. Also check your blood pressure medications; nattokinase may lower your blood pressure further.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
Nattokinase is worth a conversation with your pharmacist if you're interested in cardiovascular support—but only if you don't take blood thinners or blood pressure medications. If you do take either, this product carries real risks.
Pregnant or breastfeeding? This one isn't for you.

Nattokinase by Ortho Molecular Products Drug Interactions
HelloPharmacist Interaction Report
Nattokinase by Ortho Molecular Products interacts with blood pressure medications and blood thinners—both moderate-severity concerns.
The ingredient nattokinase has blood-thinning and clot-dissolving effects that can amplify the action of anticoagulants and antiplatelets like warfarin, dabigatran, clopidogrel, and aspirin. There's a documented case of brain bleeding in a patient who combined nattokinase with low-dose aspirin.
Adding nattokinase to these drugs raises your bleeding risk.
Read the full breakdown — every affected drug type, severity by severity
Nattokinase may also lower blood pressure on its own, so combining it with antihypertensive medications—such as ACE inhibitors, beta-blockers, calcium channel blockers, or diuretics—could theoretically increase the drop in blood pressure and cause dizziness or weakness.
Altogether, these interactions span 287 individual medications. If you take any blood thinner or blood pressure medication, use the checker below to look up your exact drugs before starting this product.

Anticoagulant/Antiplatelet Drugs
Nattokinase might increase the risk of bleeding when used with anticoagulant/antiplatelet drugs.
Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
Antihypertensive Drugs
Theoretically, nattokinase might increase the risk of hypotension when used with antihypertensive drugs.
Some clinical research suggests that nattokinase may reduce systolic and diastolic blood pressure.
