Nattokinase is a single active ingredient—a protein enzyme extracted from natto, a traditional Japanese fermented soybean food. It works by breaking down fibrin, a protein that helps form blood clots, which is why it's been studied for cardiovascular conditions.
The product is delivered in a softgel capsule with inactive ingredients that include rice bran oil, gelatin, glycerin, and several others that serve as fillers and preservatives.
Does it work?
Not established
The evidence for nattokinase isn't established yet in our data. We hold insufficient reliable evidence to rate it for angina, atherosclerosis, cardiovascular disease, cancer, and chronic fatigue syndrome.

How safe is it?
Well-documented data
Nattokinase is generally well tolerated in the short term, but its blood-thinning effect means caution if you have a bleeding disorder or are at risk for bleeding. Rare cases of bleeding have been reported with nattokinase use, including a serious case of brain hemorrhage.
Additionally, some people are sensitive to natto proteins and can have allergic reactions, from mild throat swelling to anaphylaxis, usually within 2 hours of taking it. The safety data doesn't support use during pregnancy or while breastfeeding.
