Here's the simple version: alteplase (tPA) is itself a powerful clot-busting drug given in the hospital, often for a heart attack, stroke, or serious blood clot. It works by actively dissolving clots. Nattokinase does a milder version of the same thing, breaking down fibrin (the mesh that holds clots together) and gently thinning the blood. Stack the two together and you're pushing in the same direction, so the worry is too much clot-busting and a higher chance of bleeding, including dangerous bleeding in the brain.

The evidence here is on the weaker side (mostly lab and animal data, plus one case report with aspirin), so this isn't a proven event. But because tPA is such a high-stakes drug, this is worth flagging. Always tell your hospital team and pharmacist about any supplement like nattokinase before any procedure.
