Here's the simple version: bivalirudin (Angiomax) is itself a powerful blood thinner, given through an IV during procedures like angioplasty to keep clots from forming. Nattokinase also nudges your blood toward thinner, slower clotting. Stack the two and you're pushing in the same direction, which means a higher chance of bleeding, from oozing at puncture sites to bruising, or more serious internal bleeds.

Now, the honest part: this concern comes from lab and animal data plus one case report involving aspirin, not strong human studies with bivalirudin specifically. It's not a prodrug, so there's no metabolism trick here, just two clot-fighters adding up. Since bivalirudin is used in tightly monitored hospital settings, please tell your care team about any nattokinase before a procedure rather than deciding on your own.
