HelloPharmacist Scorecard of Krill Oil 500 mg by NOW
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
NOW's Krill Oil contains 5 active ingredients: omega-3 fatty acids (a blend including eicosapentaenoic acid and docosahexaenoic acid), astaxanthin, phospholipids, and the krill oil extract itself (NKO). Omega-3s are the heart of the product — they're the polyunsaturated fats your body uses for cell health and inflammation management.
Astaxanthin is a red carotenoid antioxidant from krill. The inactive ingredients are bovine gelatin (the capsule), glycerin, and water.
Does it work?
Moderate evidence
The data we hold shows krill oil is possibly effective for dry eye symptoms. For other conditions the product might be marketed for — aging skin, Alzheimer disease, cancer, high cholesterol, or cognitive function — the evidence isn't established enough to rate reliably.
Astaxanthin is also listed with insufficient evidence for aging skin, age-related cognitive decline, macular degeneration, Alzheimer disease, carpal tunnel, and athletic performance in the data we hold.
How safe is it?
Well-documented data
Both krill oil and astaxanthin are generally well tolerated, though long-term safety data for astaxanthin are limited. With krill oil at doses up to 4 grams daily, the most common side effects are mild and gastrointestinal: bloating, decreased appetite, diarrhea, fishy burps, flatulence, heartburn, nausea, and stomach discomfort.
Headache, tiredness, and dizziness have been reported rarely. These effects tend to be milder with krill oil than with fish oil.
Astaxanthin can cause abdominal pain, diarrhea, and red-colored stools; at higher doses (40 mg), severe stomach pain has been reported in a couple of cases. For pregnancy, not enough safety data exists for either ingredient — the safety data advises against astaxanthin during pregnancy.
For krill oil, there isn't enough reliable data, so talk with your doctor before using it while pregnant or breastfeeding. If you have a shellfish allergy, avoid krill oil.
If you have a bleeding disorder, use caution with krill oil.
Meds to double-check
Moderate interaction found
Before taking this krill oil, check if you're on any blood thinners or antiplatelet drugs (warfarin, aspirin, clopidogrel) — there's a Moderate risk of increased bleeding. Also double-check diabetes medications, as krill oil may theoretically increase low blood sugar risk.
If you take drugs broken down by your liver's CYP3A4 or CYP2B6 enzymes, astaxanthin could theoretically lower their levels, though this hasn't been seen in humans yet — ask your pharmacist to verify your specific medications.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Krill oil may help with dry eye, but it's primarily an omega-3 supplement. It's generally well tolerated but interacts with blood thinners and diabetes drugs, and may theoretically affect how your liver processes certain medications.
Check your exact prescriptions before starting — especially if you take anticoagulants, antiplatelets, or diabetes medications. Talk with your pharmacist or doctor.

Formula
Krill Oil is known for its unique fatty acid (FA) profile, which includes EPA and DHA, plus phospholipids. In addition, krill oil naturally has astaxanthin, a powerful free radical scavenger and immune-supporting carotenoid. Clinical studies have shown that NKO krill oil can help to support joint comfort, healthy blood lipid levels already within normal range, and may help to maintain healthy feminine balance. The phospholipid-bound forms of EPA and DHA from krill oil have also demonstrated exceptionally high bioavailability.

Krill Oil 500 mg by NOW Drug Interactions
HelloPharmacist Interaction Report
NOW's Krill Oil 500 mg contains two ingredients with documented interactions: the krill oil itself (NKO) and astaxanthin.
The most serious concern is with blood thinners and antiplatelet drugs like warfarin or aspirin. High doses of krill oil's omega-3 fatty acids can reduce platelet stickiness, which might increase bleeding risk if combined with these medications — this is a Moderate interaction.
Read the full breakdown — every affected drug type, severity by severity
Astaxanthin in this product may theoretically speed up how your liver breaks down certain medications. In lab studies, it induced activity of two liver enzymes (CYP3A4 and CYP2B6), which could lower levels of drugs metabolized by these pathways.
Neither effect has been observed in humans yet, but it's worth checking. This is also rated Moderate.
Krill oil is also documented to theoretically increase low blood sugar risk (hypoglycemia) if you take diabetes medications, though a small study showed no glucose reduction in type 2 diabetes patients. We couldn't check three other ingredients in this product — eicosapentaenoic acid, docosahexaenoic acid, and phospholipids — so any interaction data on those isn't on file with us.
Altogether, these interactions span 827 individual medications. Before you start this krill oil, search your exact prescriptions in the tool below.

For each ingredient in Krill Oil 500 mg with known interactions, here are the types of medications they can affect. Open any type for the detail — or search your exact drug in the checker above.
Astaxanthin
Cytochrome P450 2B6 (Cyp2B6) Substrates
Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP2B6.
In vitro research shows that astaxanthin induces cytochrome CYP2B6 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Omega-3 Fatty Acids
Anticoagulant/Antiplatelet Drugs
Theoretically, DHA may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Although some clinical evidence suggests that DHA might reduce collagen-stimulated platelet aggregation and thromboxane release, most clinical evidence suggests that DHA alone does not affect blood clotting. However, theoretically, when given in combination with EPA as fish oil, concomitant use with anticoagulant or antiplatelet drugs (including aspirin) might increase risk of bleeding.
Antidiabetes Drugs
Theoretically, taking DHA with antidiabetes drugs might reduce the effects of these medications.
In people with type 2 diabetes, including those taking oral hypoglycemic medications, DHA seems to increase fasting blood glucose levels.
Antihypertensive Drugs
Theoretically, taking DHA with antihypertensive drugs might increase the risk of hypotension.
Fish oils containing DHA can lower blood pressure and might have additive effects in patients treated with antihypertensives; use with caution.
NKO
Anticoagulant/Antiplatelet Drugs
Theoretically, krill oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Taking high doses of omega-3 fatty acids from fish oil can modestly decrease platelet aggregation. Since krill oil also contains these fatty acids, taking high doses of krill oil might also inhibit platelet aggregation. Theoretically, taking high doses of krill oil with antiplatelet or anticoagulant drugs might increase the risk of bleeding. However, the omega-3 content of krill oil is much lower than that of fish oil.
Antidiabetes Drugs
Theoretically, taking krill oil with antidiabetes drugs might increase the risk of hypoglycemia.
Some research in animals and humans shows that krill oil might lower blood glucose levels. However, a small study shows that taking krill oil does not reduce blood glucose levels in patients with type 2 diabetes, most of whom were taking antidiabetes drugs.
