The capsule itself is vegetarian, and the only other ingredient listed is a plant-based fiber.

The evidence for nattokinase isn't yet established for the conditions studied, according to the data we hold. Angina, atherosclerosis, cancer, cardiovascular disease, and chronic fatigue syndrome all show insufficient reliable evidence to rate how well this ingredient works for them.
That doesn't mean it's ineffective — it means the clinical trials to date haven't provided a clear answer.
How safe is it?
Well-documented data
Nattokinase is generally well tolerated in short-term use, but its blood-thinning effect means you need to be cautious if you have any bleeding risk or bleeding disorder. The main concern is rare but serious: hemorrhage, including a reported case of brain bleeding when nattokinase was combined with aspirin.
It can also trigger allergic reactions in people sensitive to natto or its components; symptoms can include throat and airway swelling and have appeared within 2 hours of ingestion in some cases. The safety data advises against nattokinase during pregnancy and breastfeeding.

Nattokinase might increase the risk of bleeding when used with anticoagulant/antiplatelet drugs.
Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
Antihypertensive Drugs
Theoretically, nattokinase might increase the risk of hypotension when used with antihypertensive drugs.
Some clinical research suggests that nattokinase may reduce systolic and diastolic blood pressure.
