Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
This syrup contains five active ingredients: concentrated extracts of white pine, spikenard, black cherry, bloodroot, and osha. These are traditionally used in throat and cough preparations.
The product also includes inactive ingredients — evaporated cane juice, purified water, vegetable glycerine, and U.S. Pharmacopoeial alcohol — which serve as sweetener, solvent, and preservative in the liquid formulation.
Does it work?
Not established
The evidence for most uses is not well established. Black cherry is rated insufficient evidence for cough and common cold.
Spikenard is also insufficient evidence for asthma and common cold. Bloodroot shows possibly effective evidence for dental plaque and gingivitis, though those aren't typical uses for a throat syrup.
For the throat and cold symptoms this product is marketed for, the facts we hold do not establish effectiveness.
How safe is it?
Well-documented data
This product contains ingredients with notable safety cautions. Bloodroot is potentially toxic and corrosive — we don't recommend self-treatment with it.
Black cherry contains compounds that can form cyanide; used in small amounts in commercial products it's generally considered safe, but large amounts can cause cyanide toxicity, which can be fatal. Spikenard has little modern safety data available, so use it cautiously and for short periods only.
For pregnancy, spikenard and bloodroot are not recommended — spikenard is possibly unsafe, and bloodroot is likely unsafe. Black cherry is also likely unsafe in pregnancy.
For breastfeeding, all three have insufficient or unsafe data, so avoid use. If you're pregnant or nursing, talk with your doctor or pharmacist before taking this product.
Meds to double-check
Moderate interaction found
Check with your doctor or pharmacist before taking this product if you take any medications broken down by cytochrome P450 3A4 enzymes (a Moderate interaction risk from black cherry) — examples include certain cholesterol, heart, and blood pressure drugs. Also clear it if you take diuretic drugs (water pills), as spikenard may enhance their effect and increase fluid loss.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
This syrup is a blend of traditional throat herbs, but the evidence that it works for cough or cold isn't established in our data. It carries a Moderate interaction risk with certain prescription drugs broken down by your liver, and a Minor interaction with water-pill diuretics.
It also contains ingredients — bloodroot and black cherry — with serious safety concerns, especially in larger amounts and during pregnancy or breastfeeding. Run your medications through our interaction checker before starting, and discuss this product with your own doctor or pharmacist to see if it's right for you.

Osha Root Rocky Mountain Throat Syrup by Herbs, Etc. Drug Interactions
HelloPharmacist Interaction Report
Osha Root Rocky Mountain Throat Syrup by Herbs, Etc. has documented interactions with medications, mainly through its spikenard and black cherry content.
The most serious concern is a Moderate interaction: black cherry may inhibit an enzyme (cytochrome P450 3A4) that your liver uses to break down certain drugs, which could raise their levels in your blood. This has been seen in lab studies but not yet reported in humans.
Read the full breakdown — every affected drug type, severity by severity
Spikenard carries a Minor interaction with diuretic drugs (water pills). Spikenard is thought to increase sweating and fluid loss, which could enhance how diuretics work and potentially lead to excessive fluid loss and electrolyte imbalance.
We could not check white pine or balsam poplar — we hold no interaction data for them. Bloodroot, another ingredient here, has no interactions documented in our data, though it does carry separate safety concerns noted below.
Altogether, these interactions span 718 individual medications. Use the search tool on this page to check whether any of your exact prescriptions are affected.
