Lactobacillus acidophilus is generally well tolerated in healthy adults and children. The most common side effects from probiotics are mild gastrointestinal ones — bloating, gas, abdominal discomfort, and diarrhea — but these are uncommon.

FOS can cause gas and bloating, especially at higher doses, though studies suggest doses under 10 grams daily don't increase these problems more than a placebo does. One small concern: live Lactobacillus acidophilus has rarely caused serious infection in people with weakened immune systems or certain medical conditions, so check with your doctor before starting if you have compromised immunity or are hospitalized.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
