Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
Probiotic 40 Billion contains 6 active ingredients: several strains of live beneficial bacteria (Lactobacillus acidophilus, Lactobacillus plantarum, Lactobacillus paracasei, and Bifidobacterium lactis), plus a proprietary blend of additional probiotic bacteria, a marine polysaccharide complex, and fructooligosaccharide. The FOS acts as a prebiotic — food for the beneficial bacteria — while the live cultures themselves are meant to support your gut flora.
The inactive ingredients are cellulose, rice maltodextrin, and L-leucine, which serve as fillers and capsule binders.
Does it work?
Moderate evidence
Evidence exists for some of what these probiotics may do. Lactobacillus acidophilus is rated possibly effective for irritable bowel syndrome (IBS), antibiotic-associated diarrhea, bacterial vaginosis, and H. pylori infection.
For the other bacterial strains and the fructooligosaccharide in this product, the data we hold doesn't establish effectiveness — the evidence is either insufficient or not yet rated. If you're considering this supplement for a specific condition, talk it over with your doctor or pharmacist to see whether the research supports its use for your situation.
How safe is it?
Well-documented data
Lactobacillus acidophilus is generally well tolerated in healthy adults and children. The most common side effects from probiotics are mild gastrointestinal ones — bloating, gas, abdominal discomfort, and diarrhea — but these are uncommon.
FOS can cause gas and bloating, especially at higher doses, though studies suggest doses under 10 grams daily don't increase these problems more than a placebo does. One small concern: live Lactobacillus acidophilus has rarely caused serious infection in people with weakened immune systems or certain medical conditions, so check with your doctor before starting if you have compromised immunity or are hospitalized.
For pregnancy, L. acidophilus is rated possibly safe, but we hold no specific data on the other ingredients' use during pregnancy or breastfeeding — talk with your doctor about whether it's right for you.
Meds to double-check
Moderate interaction found
Before you take Probiotic 40 Billion, double-check any antibiotic drugs you're on — the live bacteria can be killed if you take them together, making the probiotic less effective. Timing them a few hours apart helps.
No other medication interactions are documented for the ingredients we could check.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
This probiotic may help with certain gut and digestive issues if the science pans out for you — especially if you've got IBS or are dealing with antibiotic-related diarrhea. The main thing to watch: space it apart from antibiotics, since they'll kill the live cultures.
If you have a weakened immune system or serious health concerns, check with your own doctor first. Most people tolerate it well, but some gas and bloating at the start is normal.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
