Probiotic 40 Billion contains 6 active ingredients: four different live probiotic bacteria (Lactobacillus paracasei, Lactobacillus acidophilus, Lactobacillus plantarum, and Bifidobacterium lactis), fructooligosaccharide (a prebiotic fiber that feeds beneficial bacteria), and a marine polysaccharide complex. The capsule also holds three inactive ingredients — cellulose, rice maltodextrin, and L-leucine — which are fillers and binders that help hold the capsule together and preserve the active ingredients.

The evidence for Lactobacillus acidophilus (one of the active ingredients) is mixed. It's rated possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
For atopic disease (allergic conditions), the evidence suggests it's possibly ineffective, and for acute respiratory distress syndrome, there isn't enough reliable evidence to rate it either way. For the other bacterial strains and the fructooligosaccharide in this product, the evidence in the data we hold isn't established — we don't have effectiveness ratings for them.

Lactobacillus acidophilus is generally well tolerated in healthy adults and children. That said, because these are live organisms, there is a small theoretical risk of infection in vulnerable patients — cases of bacteremia, sepsis, and other serious infections have been reported, though most are thought to happen when bacteria move from other parts of the body where they naturally occur.
Your doctor should weigh this if you're immunocompromised or have serious underlying illness. Gastrointestinal side effects from the probiotics or the prebiotic fiber are uncommon but can include belly discomfort, bloating, flatulence, diarrhea, and burping.
Fructooligosaccharide is generally well tolerated in food amounts but can cause gas and bloating, especially at higher doses. For both ingredients, safety data at supplement doses are limited, so check with your doctor before use.
Pregnancy safety for Lactobacillus acidophilus is rated possibly safe, but data for the other ingredients in pregnancy or breastfeeding aren't on file — talk with your healthcare provider for personalized advice if you're pregnant or nursing.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
