Pycnogenol is generally well tolerated in studies, though long-term safety hasn't been fully worked out. The most common side effects are stomach complaints, dizziness, and vertigo—a small number of trial participants also reported headache.

One person had chest pain and another had worsening heart failure, but both had existing heart conditions and it's unclear whether the supplement was the cause. Theoretically, maritime pine bark extract might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Clinical research suggests that maritime pine bark extract inhibits platelet aggregation. However, the clinical significance of this effect is unclear.
