Long-term safety at high doses isn't fully established. A few isolated reports mention headache, fatigue, and memory loss in people on peritoneal dialysis, and one case of a skin rash that cleared after stopping the supplement.
The safety data advises against concentrated supplements during pregnancy and while breastfeeding because there isn't enough information to know either way — talk with your doctor or pharmacist for personalized guidance if you're pregnant or nursing.

Resveratrol also theoretically affects how your liver metabolizes a range of other medications. In vitro research shows it can inhibit multiple liver enzyme systems (CYP2C19, CYP1A2, CYP3A4, and CYP2E1), which might increase blood levels of drugs broken down by those pathways.
However, most of these interactions haven't been observed in actual people — one clinical study found that even at high doses (3000 mg daily), resveratrol didn't require dose adjustments to CYP3A4 drugs. A pharmacokinetic study did show that 500 mg daily for 10 days increased chlorzoxazone levels by about 54%, suggesting the CYP2E1 interaction may be real at certain doses.

Also, a pharmacokinetic study shows that taking resveratrol 500 mg daily for 10 days prior to taking a single dose of chlorzoxazone 250 mg increases the maximum concentration of chlorzoxazone by about 54%, the area under the curve of chlorzoxazone by about 72%, and the half-life of chlorzoxazone by about 35%. Chlorzoxazone is used as a probe drug for CYP2E1.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.
