Atovaquone, Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.

Here's the gist with Malarone (atovaquone and proguanil) and resveratrol. The proguanil part of this combo is actually a prodrug, meaning your body uses an enzyme called CYP2C19 to convert it into its active, malaria-fighting form (cycloguanil). In lab dishes, resveratrol has been shown to slow that same enzyme down. In theory, that could mean a bit less of the active form gets made, which could in turn make the malaria protection slightly weaker.

That said, I want to keep this in perspective. This is only seen in test-tube studies, not in real people, so it's very theoretical. The atovaquone half still works on its own, too. Since malaria prevention is not something to leave to chance, just run your resveratrol plans by your pharmacist or doctor first.

What Is Atovaquone and Proguanil Used For?

Brand name: Malarone

Atovaquone and proguanil is used for two purposes: preventing Plasmodium falciparum (P. falciparum) malaria before and during travel to areas where it is present, and treating acute, uncomplicated P. falciparum malaria once infection has occurred. This includes areas where other antimalarials such as chloroquine, mefloquine, halofantrine, or amodiaquine may have unacceptable failure rates due to drug resistance.

These uses apply across the supported products: Atovaquone and Proguanil Hydrochloride tablets, Atovaquone and Proguanil Hydrochloride Pediatric tablets, and Malarone tablets. Pediatric formulations are dosed by body weight for children; adult-strength formulations are used for adults and heavier children.
