Super Omega-3 Plus contains 8 active ingredients. The omega-3 fatty acids eicosapentaenoic acid and docosahexaenoic acid are the marine oils that form the backbone of this supplement.
You're also getting sesame seed lignan extract, natural astaxanthin (a carotenoid pigment from marine sources), and polyphenols — plant compounds with antioxidant properties — sourced from olive extract (hydroxytyrosol, tyrosol, oleuropein, and verbascoside). The product is rounded out with phospholipids to help with absorption.
The inactive ingredients include fish oil and krill oil concentrates, gelatin capsule material, glycerin, beeswax, water, silica, natural flavor, olive oil, caramel color, mixed tocopherols (vitamin E), maltodextrin, and rosemary extract.
Does it work?
Moderate evidence
For sesame seed lignan extract, the data shows it's possibly effective for high blood pressure (hypertension), but possibly ineffective for cough and there isn't enough reliable evidence yet to rate it for nonalcoholic fatty liver disease, HIV/AIDS-related wasting, Alzheimer disease, or anemia of chronic disease. For natural astaxanthin, we don't have enough reliable evidence to rate its effectiveness for age-related cognitive decline, age-related macular degeneration, aging skin, Alzheimer disease, athletic performance, or carpal tunnel syndrome.

How safe is it?
Well-documented data
Sesame seed lignan extract is generally well tolerated in food amounts, but it's a major allergen — up to 0.5% of the U.S. population reports a sesame allergy, and about 0.23% meet criteria for a true IgE-mediated allergic reaction. Allergic reactions can be skin-related (swelling, hives, itching, redness) or affect the respiratory system.
One trial participant experienced diarrhea. During pregnancy, there isn't enough data to know whether concentrated sesame supplements are safe, so talk with your doctor before using this product.
The same applies to breastfeeding — safety data are limited. Natural astaxanthin is generally well tolerated, though long-term safety data are limited.
The most common side effects reported orally are abdominal pain, diarrhea, and red-colored stool. At lower doses (6 mg daily), a few people reported increased bowel movements and red stool; at higher doses (40 mg daily), two patients developed severe stomach pain.
The safety data advise against astaxanthin in pregnancy and breastfeeding due to insufficient information.

Before taking this product, double-check with your doctor or pharmacist if you take antidiabetes drugs — the sesame content may lower blood sugar further and cause hypoglycemia (low blood sugar). Also flag any antihypertensive drugs (blood pressure medications), since sesame may increase the risk of excessively low blood pressure.
If you take drugs metabolized by your liver through the CYP3A4, CYP2B6, or CYP2C9 pathways, or if you take tamoxifen, discuss this product with your healthcare provider first.

Super Omega-3 Plus by Life Extension contains sesame seed lignan extract and natural astaxanthin, both of which have documented interactions with medications.
The most serious concern is a moderate risk of low blood sugar (hypoglycemia) if you take antidiabetes drugs — sesame oil can lower blood sugar on its own, and combining it with diabetes medication may push levels too low.

Sesame also interacts moderately with blood pressure medications (antihypertensive drugs), which may increase the risk of excessively low blood pressure. It theoretically affects how your liver metabolizes certain drugs processed through the CYP2C9 pathway, and may interfere with tamoxifen (Nolvadex), a breast cancer medication.
At a minor level, sesame may alter how your body transports some drugs via a cellular pump called P-glycoprotein.
Astaxanthin theoretically may decrease blood levels of drugs metabolized through the CYP3A4 and CYP2B6 pathways — both handled by your liver. These effects haven't been confirmed in humans, but in test-tube studies the ingredient induced these enzyme pathways.

natural Astaxanthin
Cytochrome P450 2B6 (Cyp2B6) Substrates
Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP2B6.
In vitro research shows that astaxanthin induces cytochrome CYP2B6 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Sesame seed Lignan extract
Antidiabetes Drugs
Taking sesame oil with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical studies show that sesame oil can decrease plasma glucose and glycated hemoglobin (HbA1c) levels. Some clinical research in patients taking glibenclamide shows that using sesame oil or a blend of sesame oil and rice bran oil in place of other oil for cooking reduces plasma glucose more than glibenclamide alone. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Antihypertensive Drugs
Taking sesame oil with antihypertensive drugs might increase the risk of hypotension.
Clinical research shows that replacing other cooking oil with sesame oil can lower systolic blood pressure (SBP) and diastolic blood pressure (DBP) in patients with or without hypertension. There is also some evidence that sesame oil has additive effects in patients also taking atenolol, nifedipine, and/or hydrochlorothiazide. In patients using nifedipine, using a blend of sesame oil and rice bran oil for cooking reduces both SBP and DBP more than nifedipine alone.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, sesame might increase the levels and clinical effects of CYP2C9 substrates.
In vitro, sesame inhibits CYP2C9. However, this interaction has not been reported in humans.
Tamoxifen (Nolvadex)
Theoretically, sesame might interfere with tamoxifen.
In animal research, sesame seed reduces the tumor-inhibitory effect of tamoxifen by reducing apoptosis and increasing proliferation. This interaction has not been reported in humans.
P-Glycoprotein Substrates
Theoretically, sesame might alter the transport of P-glycoprotein substrates.
In vitro research suggests that sesamin, a constituent of sesame, can inhibit the multi-drug transporter protein, P-glycoprotein. However, this interaction has not been reported in humans.
