Orally, sweet wormwood is generally well tolerated. The most common side effects are nausea and vomiting.

Rarely, it can cause liver damage (hepatotoxicity) — one reported case involved a person taking a high dose of artemisinin (a component of sweet wormwood) who developed hepatitis, though causation wasn't certain. One person also reported a mild allergic reaction (rash and cough that resolved quickly), and people allergic to it have experienced numbness of the tongue and throat itching when taking low doses under the tongue.

Avoid sweet wormwood during pregnancy because of possible harm to the developing baby. There isn't enough safety information to use it while breastfeeding — talk with your doctor or pharmacist about whether it's right for you.

Cytochrome P450 2B6 (Cyp2B6) Substrates

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP2B6.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP2B6, possibly increasing CYP2B6 activity by 1.6-fold. However, Sweet Annie extract seems to inhibit the activity of CYP2B6 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP2B6 substrates.
