Tansy contains thujone, which is poisonous, so internal use is generally discouraged. The safety data advises against use in pregnancy — tansy has been used traditionally to bring on menstruation or abortion.

Safety is not established while breastfeeding, and the thujone content makes it unsafe during lactation. Adverse effects from thujone toxicity can include rapid heartbeat, irregular heartbeat, rapid breathing, severe stomach upset, vomiting, dilated pupils, liver damage, kidney damage, and in severe cases, death.

Fatalities have been reported from as little as 10 drops of tansy oil, occurring within 1 to 3.5 hours after ingestion. Topical contact may cause skin reactions like eczema or photosensitivity (sun sensitivity) in some people.
