Every active ingredient lists its own amount on the label.

Why this rating?
  • The label discloses an exact amount for 1 of its 1 active ingredient.
  • No proprietary blends here — you can verify the dose of every single component.

This product contains one active ingredient: wildcrafted Tongkat Ali root liquid extract from Eurycoma longifolia. It's delivered in a liquid form using menstruum (a plant-based solvent) to carry the extract.

The root is the part used in traditional herbal medicine and has been studied for its potential effects on sexual function and male health.

Moderate evidence
Evidence for Intended Use · database check
By FDA rules, dietary supplements can’t claim to treat, cure, or prevent disease — so labels speak in careful marketing language. We discern each product’s intended use from its name, label claims, and label statements, then grade the clinical evidence for that use. How these ratings are computed

This product doesn't appear to be marketed for a specific use, so we graded Eurycoma Longifolia's overall clinical evidence instead.

Moderate

Some clinical evidence supports Eurycoma Longifolia for:

Why this rating?
  • We looked at the product name, claims, and label statements and couldn't find a stated purpose to grade.
  • Since the label doesn't commit to one use, we graded the ingredient's overall clinical evidence instead.
  • On file: Sexual desire — rated "Possibly Effective" (Natural Medicines).

The evidence for Tongkat Ali's effectiveness is mixed depending on what you're using it for. Sexual desire is rated possibly effective — some research supports this use.

Athletic performance, though, is rated possibly ineffective. For back pain, osteoporosis, and diabetes, we don't have enough reliable evidence to rate whether it works.

If you're considering it for sexual function, talk with your pharmacist or doctor about whether the current evidence fits your situation.

How safe is it?

Well-documented data

Adverse-effect, pregnancy, and general safety data are on file for most of these ingredients.

Why this rating?
  • We hold adverse-effect (side-effect) data for 1 of the 1 matched ingredient.
  • Pregnancy & breastfeeding safety ratings cover 1 of 1.
  • General safety write-ups exist for 1 of 1.
  • Remember: this measures how much safety information exists. Thin data is not the same as being safe.

Tongkat Ali is generally well tolerated in short-term studies, but we don't have solid data on long-term safety or product quality across different brands. There's an important concern about hormonal effects: the supplement can raise testosterone levels, especially in men with low baseline testosterone.

If testosterone goes beyond normal, you could experience acne, insulin resistance, liver strain, or other testosterone-related side effects. Because of possible hormonal effects and limited safety data, you should avoid this supplement during pregnancy and while breastfeeding.

Why this rating?
  • 1 of the 1 matched ingredient can interact with medications — Eurycoma Longifolia.
  • The most serious interaction on file is rated Moderate.

Check with your pharmacist before starting if you take propranolol (Inderal) or any beta-blockers, as this supplement can reduce their effectiveness. Also double-check if you're on testosterone replacement therapy or any medications that rely on liver enzymes (CYP1A2, CYP2C19, or CYP2A6) to be cleared from your body.

The bottom line

Scorecard at a glanceFully disclosed formula with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.

Tongkat Ali may be worth considering if you're interested in its possibly effective use for sexual desire and have low testosterone, but only after checking with your pharmacist or doctor — especially if you take propranolol, testosterone therapy, or medications processed through liver enzymes. Long-term safety isn't well established, and the hormonal effects mean it's not appropriate during pregnancy or breastfeeding.

Propranolol (Inderal)

Eurycoma longifolia can reduce the levels and clinical effects of propranolol.
A small clinical study in healthy persons shows that taking a single dose of a water-based Eurycoma longifolia extract 200 mg, in combination with a single dose of propranolol 80 mg, reduces the propranolol area under the curve (AUC) by 29%, reduces the peak concentration by 42%, and increases time to peak concentration by 86% when compared with control. Since the elimination half-life of propranolol did not change, it seems that Eurycoma longifolia alters the kinetics of propranolol by decreasing its absorption in the gut, and not by altering its metabolism. It is not known if separating administration will prevent this interaction.

Likelihood Probable Evidence A
Cytochrome P450 1A2 (Cyp1A2) Substrates

Theoretically, Eurycoma longifolia might increase levels CYP1A2 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.

Likelihood Possible Evidence D
Cytochrome P450 2A6 (Cyp2A6) Substrates

Theoretically, Eurycoma longifolia might increase levels of CYP2A6 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2A6 enzymes. This effect has not been reported in humans.

Likelihood Possible Evidence D
Cytochrome P450 2C19 (Cyp2C19) Substrates

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Likelihood Possible Evidence D
Testosterone

Theoretically, Eurycoma longifolia may further increase levels of testosterone.
A clinical study in aging males with testosterone levels below 300 ng/dL shows that taking a specific water extract of Eurycoma longifolia roots (Physta; Biotropics Malaysia) 100-200 mg daily with breakfast for 12 weeks increases total testosterone levels by 8% to 11% when compared with placebo. It is unclear whether this increase would occur in individuals with normal testosterone levels.

Tongkat Ali by Herbal Terra: Common Questions

Does Tongkat Ali by Herbal Terra interact with any medications?
Yes. Based on its ingredients, Tongkat Ali has a known interaction with 248 medications. Use the checker to see how it interacts with a specific drug.
How can one product interact with so many drugs?
Tongkat Ali contains a single active ingredient, and that one ingredient can interact with many different medications on its own. We check it against each medication and show the mechanism responsible.
Where does this information come from?
The product label data comes from the NIH Dietary Supplement Label Database (DSLD); the interaction data is built on the Natural Medicines database and reviewed by HelloPharmacist pharmacists.
What's Tongkat Ali actually used for?
The research suggests it's possibly effective for boosting sexual desire. For athletic performance, the evidence leans toward it probably not working. For back pain, osteoporosis, and diabetes, we just don't have enough reliable studies yet to say either way.
Can I take this while pregnant or breastfeeding?
No — the safety data advises against it during pregnancy because of possible hormonal effects. While breastfeeding, there's no reliable safety information, so it's best to avoid it then too. Talk with your doctor or pharmacist if you have questions about what's safe for your situation.
What are the side effects I should watch for?
Because Tongkat Ali can raise testosterone levels, especially if yours are low to begin with, watch for acne, changes in blood sugar control, or signs of liver problems if levels climb too high. The supplement is generally tolerated in the short term, but long-term safety isn't as clear.
Is this safe to take long-term?
The research we have focuses on short-term use, and long-term safety data is limited. We also can't guarantee product quality across different brands, so it's worth talking with your pharmacist about how long you plan to use it and whether it's right for you.
Does this interact with my blood pressure medication?
Yes, if you take propranolol (Inderal), Tongkat Ali can reduce how well it works by lowering its levels in your blood by about 29%. If you're on any blood pressure or heart medication, check with your pharmacist first.
What's actually in this liquid?
The active ingredient is Tongkat Ali root extract from the Eurycoma longifolia plant. It's suspended in menstruum, which is a plant-based solvent that carries the extract in liquid form.
