Wild cherry is generally considered safe in the small amounts found in commercial cough products, but there's a serious catch: the plant contains compounds that can form cyanide when ingested in large quantities. Large amounts can cause cyanide toxicity, which can be fatal — a documented case involved a person who consumed about 300 grams of wild cherries steeped in alcohol and presented with headache, nausea, vomiting, confusion, severe breathing trouble, and eventually lost consciousness.

Because of these cyanide-forming compounds, safety has not been established during pregnancy or breastfeeding, and the data advises against use in both. Stick to small, typical doses only.

Cytochrome P450 3A4 (Cyp3A4) Substrates

In vitro research suggests that wild cherry can inhibit cytochrome P450 3A4 (CYP3A4) enzymes. Theoretically, wild cherry might increase levels of drugs metabolized by CYP3A4. However, so far, this interaction has not been reported in humans.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and others.
