Taken by mouth or applied inside the vagina, yarrow is generally well tolerated. However, when applied to the skin, it can trigger allergic or eczema-like dermatitis in some people, due to compounds called sesquiterpene lactones.

It's also been reported to cause reactions to sun exposure, airborne contact dermatitis, and in rare cases, breathing problems. One person in a trial reported acne got worse after vaginal use.

Because safety data are limited and allergic reactions can happen, yarrow is best used short-term and under guidance. Do not use in pregnancy—it's traditionally avoided because it may affect the uterus.

Breastfeeding mothers should avoid it too, as there isn't enough safety information.

Theoretically, taking yarrow with lithium might increase the levels and adverse effects of lithium.
Animal research shows that yarrow has diuretic activity. Theoretically, due to these potential diuretic effects, yarrow might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
