What happens

Increased dextroamphetamine or lisdexamfetamine (prodrug of dextroamphetamine) exposure and an increased risk of serotonin syndrome

Interaction Deep Dive

The concomitant use of dextroamphetamine or lisdexamfetamine (prodrug of dextroamphetamine) and CYP2D6 inhibitors may increase the exposure of dextroamphetamine, the active metabolite of lisdexamfetamine compared to the use of the drug alone and also increase the risk of serotonin syndrome. If concomitant use of dextroamphetamine or lisdexamfetamine with CYP2D6 inhibitors is clinically warranted, initiate with lower doses and monitor patients for signs and symptoms of serotonin syndrome particularly during dextroamphetamine or lisdexamfetamine initiation and after a dosage increase. Serotonin syndrome symptoms may include mental status changes, autonomic instability, seizures, and/or gastrointestinal symptoms, neuromuscular symptoms. If serotonin syndrome occurs, discontinue dextroamphetamine or lisdexamfetamine and the CYP2D6 inhibitor. Consider an alternative drug that does not inhibit CYP2D61.
