How we grade severity & evidence

Severity levels

  • Contraindicated: These should generally not be used together.
  • Major: Potentially serious — often needs a change or close monitoring.
  • Moderate: Can be significant — usually manageable with monitoring.
  • Minor: Usually limited clinical impact.

Evidence grades

  • Established: Well documented — supported by controlled studies or strong clinical data.
  • Probable: Good supporting evidence, though not definitively proven.
  • Suspected: Some evidence suggests this interaction, but it is not well established.
  • Possible: Limited or conflicting evidence; the interaction may occur.
  • Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.

Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.

Of 179 documented Atazanavir interactions, 134 are rated major — including this one.

What happens

Increased lenacapavir exposure, increased atazanavir exposure and an increased risk of atazanavir-related adverse reaction

Interaction Deep Dive

Concomitant administration of lenacapavir with atazanavir/ritonavir or atazanavir/cobicistat is not recommended. Coadministration of lenacapavir together with atazanavir/ritonavir or atazanavir/cobicistat may result in increased exposure of lenacapavir2. Lenacapavir may increase the exposure of drugs primarily metabolized by CYP3A, initiated within 9 months after the last dose of lenacapavir 12. Coadministration of lenacapavir with atazanavir (strong CYP3A, P-gp and UGT1A1 inhibitor) may increase the exposure of atazanavir and result in the increased risk of atazanavir-related adverse events 1.

Why it happens (mechanism)

Inhibition of CYP3A-mediated metabolism of lenacapavir; inhibition of CYP3A-mediated metabolism of atazanavir; inhibition of P-gp-mediated efflux transport of lenacapavir; inhibition of UGT1A1-mediated efflux transport of lenacapavir
