How we grade severity & evidence

Severity levels

  • Contraindicated: These should generally not be used together.
  • Major: Potentially serious — often needs a change or close monitoring.
  • Moderate: Can be significant — usually manageable with monitoring.
  • Minor: Usually limited clinical impact.

Evidence grades

  • Established: Well documented — supported by controlled studies or strong clinical data.
  • Probable: Good supporting evidence, though not definitively proven.
  • Suspected: Some evidence suggests this interaction, but it is not well established.
  • Possible: Limited or conflicting evidence; the interaction may occur.
  • Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.

Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.

Of 57 documented Doravirine interactions, 16 are rated contraindicated — including this one.

What happens

Reduced doravirine exposure, reduced efficacy of doravirine and an increased risk of doravirine resistance

Interaction Deep Dive

Concomitant use of doravirine and strong CYP3A inducer, such as PHENobarbital is contraindicated. Coadministration may result in reduced doravirine exposure, loss of doravirine efficacy and development of resistance. In a drug interaction study, coadministration of rifAMPin (strong CYP3A inducer) and doravirine decreased the AUC, Cmax, and Cmin of doravirine by 0.12-fold, 0.43-fold. and 0.03-fold, respectively. Before initiating doravirine, it is recommended to wait at least 4 weeks after discontinuing the CYP3A inducer21.

Why it happens (mechanism)

Induction of CYP3A-mediated doravirine metabolism
