Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the connection worth knowing about. Your body relies on a liver enzyme called CYP3A4 to break down doravirine (Pifeltro). Adrafinil turns into modafinil in your body, and modafinil can gently speed up that same enzyme. In theory, a faster enzyme could clear doravirine out of your blood a little quicker, which might lower its levels.

Why that matters with an HIV medicine: if doravirine drops too low, it may not keep the virus fully suppressed, and over time that can risk the virus becoming resistant. I want to stress this is theoretical, based on how these drugs behave rather than real-world cases, so it may not amount to much. Still, with HIV therapy you don't want to gamble. Please check with your doctor or pharmacist before pairing these.

About Doravirine

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used to treat HIV-1 infection in adults and in children and adolescents who weigh at least 35 kg (about 77 lbs). It is always used in combination with other antiretroviral medicines — never on its own.

Pifeltro is approved for two groups: people who have never taken HIV treatment before, and people who are already virologically suppressed (meaning HIV is undetectable in their blood) on a stable regimen and need to switch, provided they have no history of treatment failure or known resistance to doravirine.
