Increased risk of respiratory and CNS depression and an increased risk of serotonin syndrome
Interaction Deep Dive
Concomitant use of morphine and serotonergic CNS depressants may increase risk of respiratory depression, profound sedation, hypotension, coma, opioid overdose and serotonin syndrome. Reserve the concomitant use of morphine and CNS depressants to patients for whom alternatives are inadequate. If concomitant use is needed, use the lowest dose and shortest duration necessary to achieve treatment goals, closely monitor for sedation, respiratory depression, and signs and symptoms of serotonin syndrome. Carefully observe the patient, particularly during treatment initiation and dose adjustments. Discontinue morphine if serotonin syndrome is suspected4. The risk of opioid overdose is increased with concurrent use of morphine and CNS depressants. Consider prescribing naloxone for the emergency treatment of opioid overdose 3. If morphine/naltrexone combination is being initiated, start at one-third to one-half of the usual morphine/naltrexone dose 5. If Morphabond ER(TM) is being initiated, start at the lowest possible dose, 15 mg every 12 hours, and consider using a lower dosage of the concomitant CNS depressant 6.
