Mean Cmax and AUC of repaglinide, a CYP2C8 substrate, were increased by 1.7- and 2.4-fold, respectively, when repaglinide 0.25 mg was administered following repeated doses of teriflunomide (the active metabolite of leflunomide1), suggesting that teriflunomide is a CYP2C8 inhibitor 2. The AUC of pioglitazone, a CYP2C8 substrate, was increased 3.4-fold and the half-life increased from 6.5 to 15.1 hours when a 30-mg dose was coadministered with gemfibrozil, a strong CYP2C8 inhibitor, dosed at 600 mg twice daily for two days 3. Monitor patients taking pioglitazone and leflunomide or teriflunomide concomitantly 2; a pioglitazone dose adjustment may be necessary 3.
