Leflunomide falls under HEPATOTOXIC DRUGS — the group Buchu interacts with.

Buchu contains pulegone, a known hepatotoxin. There is some concern that buchu may adversely affect the liver, especially when the leaf is used in large doses or the oil is ingested. Theoretically, concomitant use with hepatotoxic drugs might increase the risk of liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (Tricor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo)

Here's the gist: Leflunomide is already one of the harder medications on your liver. That's why your doctor checks your liver enzymes with blood tests, especially when you first start it. Buchu enters the picture because its essential oil contains a compound called pulegone, which can also be tough on liver cells. Stack the two together and, at least in theory, you could be adding extra strain to your liver.

I want to be honest that this concern is based on how these things work, not on real-world cases, so the evidence here is fairly weak. Still, with a drug like Leflunomide, I'd play it safe. Watch for warning signs like unusual tiredness, yellowing of your skin or eyes, dark urine, or pain in your upper right belly. Please check with your pharmacist or doctor before adding Buchu.

Leflunomide tablets — sold under the brand name Arava and as generic leflunomide — are used to treat active rheumatoid arthritis (RA) in adults. Both Arava and generic leflunomide tablets are FDA-approved for this use.

Rheumatoid arthritis is an autoimmune condition in which the immune system attacks the joints, causing pain, swelling, and stiffness. Leflunomide helps calm that overactive immune response to reduce symptoms and slow joint damage.
