Baricitinib falls under HEPATOTOXIC DRUGS — the group Buchu interacts with.

Buchu contains pulegone, a known hepatotoxin. There is some concern that buchu may adversely affect the liver, especially when the leaf is used in large doses or the oil is ingested. Theoretically, concomitant use with hepatotoxic drugs might increase the risk of liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (Tricor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo)

Here's the simple version: buchu contains a compound called pulegone that can be hard on the liver, mostly when the concentrated oil or large doses of the leaf are swallowed. Baricitinib (Olumiant) can also occasionally raise liver enzymes, so the worry is that putting both together might add extra stress on your liver.

I want to be honest that this is a theoretical concern based on how these things behave, not on real cases of people taking them together. Buchu in normal food or tea amounts is unlikely to be a problem. Still, since your doctor likely checks bloodwork while you're on baricitinib, it's worth mentioning any buchu supplement or oil to your pharmacist or doctor before combining them.

Buchu is a fragrant South African shrub whose leaves are traditionally used for urinary and bladder complaints and as a diuretic. Solid human studies are lacking, so its benefits are largely unproven, and it should be avoided in pregnancy and breastfeeding. What Is Baricitinib Used For?

Brand name: Olumiant

Olumiant (baricitinib) tablets are approved for three conditions in adults. It treats moderately to severely active rheumatoid arthritis in people who haven't responded well enough to one or more TNF blocker medications. It also treats severe alopecia areata (significant hair loss from an autoimmune condition).

Olumiant is additionally approved to treat COVID-19 in hospitalized adults who need supplemental oxygen, mechanical ventilation, or ECMO (a machine that does the work of the heart and lungs). Each use has specific limitations — for example, Olumiant should not be combined with other JAK inhibitors or certain strong immunosuppressants for rheumatoid arthritis or alopecia areata.
