Eltrombopag falls under HEPATOTOXIC DRUGS — the group Buchu interacts with.

Buchu contains pulegone, a known hepatotoxin. There is some concern that buchu may adversely affect the liver, especially when the leaf is used in large doses or the oil is ingested. Theoretically, concomitant use with hepatotoxic drugs might increase the risk of liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (Tricor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo)

Here's the thing to know about pairing buchu with eltrombopag. Eltrombopag is one of those medicines that can be tough on the liver on its own, which is why your doctor checks your liver enzymes during treatment. Buchu leaves (and especially the essential oil) contain a compound called pulegone that may also stress the liver. The worry is that using both together could add up and raise the chance of liver trouble, with signs like yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness.

I want to be honest though: this is a theoretical concern based on how these things work, not solid proof from studies. A bit of buchu in food or tea is far less concerning than concentrated capsules or the oil. Still, since eltrombopag already deserves careful liver monitoring, please check with your pharmacist or doctor before adding buchu.

Buchu is a fragrant South African shrub whose leaves are traditionally used for urinary and bladder complaints and as a diuretic. Solid human studies are lacking, so its benefits are largely unproven, and it should be avoided in pregnancy and breastfeeding. About Eltrombopag

What Is Eltrombopag Used For?

Brand names: Promacta, Revolade, Alvaiz

Eltrombopag treats thrombocytopenia (low platelet counts) in several different conditions. It is used for adults and children (age 1 and older, depending on the product) with persistent or chronic immune thrombocytopenia (ITP) — a condition where the immune system destroys platelets — when other treatments like steroids or immunoglobulins haven't worked well enough. It is also used to raise platelet counts in adults with chronic hepatitis C whose low platelet count prevents them from starting or staying on interferon-based antiviral therapy.

Eltrombopag tablets (including Promacta) are also approved — alone or combined with immunosuppressive therapy — for adults and certain children (age 2 and older) with severe aplastic anemia, a serious bone marrow failure condition. Alvaiz (a specific brand) shares most of these uses but has product-specific dosing that is not interchangeable milligram-for-milligram with other eltrombopag products. The oral suspension formulation is approved for ITP and for refractory severe aplastic anemia, but is not approved for first-line severe aplastic anemia in combination with immunosuppressive therapy.
