Ofloxacin falls under HEPATOTOXIC DRUGS — the group Buchu interacts with.

Buchu contains pulegone, a known hepatotoxin. There is some concern that buchu may adversely affect the liver, especially when the leaf is used in large doses or the oil is ingested. Theoretically, concomitant use with hepatotoxic drugs might increase the risk of liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (Tricor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo)

Here's the simple version: buchu leaves (and especially the concentrated oil) contain a compound called pulegone that can be hard on the liver. Ofloxacin, like many antibiotics, is also processed by your liver and can occasionally stress it. The worry is that taking both together might add up and raise the chance of liver irritation. Watch for signs like yellowing of the skin or eyes, dark urine, belly pain on the upper right side, or unusual tiredness.

That said, this is mostly a theoretical concern based on how these things work, not on strong human studies. A pinch of buchu in tea is far less worrisome than swallowing the essential oil. Still, while you're on ofloxacin, check with your pharmacist or doctor before adding buchu.

Buchu is a fragrant South African shrub whose leaves are traditionally used for urinary and bladder complaints and as a diuretic. Solid human studies are lacking, so its benefits are largely unproven, and it should be avoided in pregnancy and breastfeeding. What Is Ofloxacin Used For?

Brand names: Oflox, Floxin, Floxin Injection, Floxin Otic

Ofloxacin is used in several different forms, each approved for specific infections. The otic (ear) solution treats outer ear infections (otitis externa) in patients 6 months and older, chronic middle ear infections with a ruptured eardrum (chronic suppurative otitis media) in patients 12 and older, and acute middle ear infections in children 1 year and older who have ear tubes. The ophthalmic (eye) solution — including the brand Ocuflox — treats bacterial conjunctivitis (pink eye) and bacterial corneal ulcers.

Ofloxacin oral tablets treat adults with mild to moderate bacterial infections including community-acquired pneumonia, skin and skin structure infections, gonorrhea, chlamydial infections of the urethra or cervix, mixed urethral/cervical infections, and acute pelvic inflammatory disease. The oral tablet is also used for acute bacterial exacerbations of chronic bronchitis and uncomplicated cystitis, but only when no other treatment options are available, due to serious risk concerns with this drug class.
