HEPATOTOXIC DRUGS
Why this applies to Baricitinib
Baricitinib falls under HEPATOTOXIC DRUGS — the group Siberian Cocklebur interacts with.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the concern with pairing Siberian cocklebur and baricitinib (Olumiant): it's not about one changing the other's blood levels. Instead, it's that both can be hard on your liver. Siberian cocklebur has been tied to real cases of liver injury, and even liver failure, because of its naturally toxic compounds. Baricitinib can also stress the liver, which is why your doctor checks your liver enzymes while you're on it.
Stacking two liver stressors together may raise your risk of liver trouble. Watch for warning signs like yellowing of the skin or eyes, dark urine, belly pain on the right side, nausea, or unusual tiredness. Please don't start or stop anything on your own. Talk with your pharmacist or doctor before combining these, especially since cocklebur can be genuinely toxic.

From our pharmacist’s overview on the Baricitinib page
“Liver strain dominates this page. Baricitinib (Olumiant), a JAK inhibitor for rheumatoid arthritis and alopecia areata, can raise liver enzymes on its own, so I'd approach these combinations warily, since nearly two thirds of the listed supplements carry their own record of liver injury, and ordinary beer and wine sit among the highest…”

About Baricitinib
What Is Baricitinib Used For?
Brand name: Olumiant
Olumiant (baricitinib) tablets are approved for three conditions in adults. It treats moderately to severely active rheumatoid arthritis in people who haven't gotten enough relief from one or more TNF blocker medications. It also treats severe alopecia areata (significant hair loss from an immune condition). For COVID-19, Olumiant is used in hospitalized adults who need supplemental oxygen, mechanical ventilation, or ECMO (a machine that takes over heart-lung function).
Olumiant is not recommended alongside other JAK inhibitors, biologic DMARDs (disease-modifying antirheumatic drugs), or potent immunosuppressants like cyclosporine or azathioprine for rheumatoid arthritis or alopecia areata.
