Tirzepatide and levothyroxine should be used together with caution. Tirzepatide delays gastric emptying and may impact absorption of drugs dependent on threshold concentrations for efficacy and with a narrow therapeutic index, such as levothyroxine1. Additionally, decreased weight-based levothyroxine dosages may be required in patients experiencing rapid weight loss from tirzepatide. In a case report, iatrogenic thyrotoxicosis following rapid weight loss from tirzepatide therapy was described in a patient receiving a stable dose of levothyroxine. Monitor patients receiving tirzepatide and levothyroxine closely (eg, every 4 to 6 weeks initially) and adjust dosage of levothyroxine if necessary 2.

a) Thyrotoxicosis following rapid weight loss from tirzepatide therapy was described in a case report involving a patient receiving a stable dose of levothyroxine. The patient was a 62-year-old male and with a history of autoimmune hypothyroidism, obesity, and type 1 diabetes receiving levothyroxine 200 mcg daily, tirzepatide 10 mg weekly, and insulin. He presented with palpitations, excessive sweating, confusion, fever, and hand tremors. Significant findings on presentation included atrial fibrillation on ECG, thyrotropin level of 0.001 milli-international units/L, free thyroxine level of 7.26 nanograms/dL, and significant weight loss (more than 36 kg in 6 months). Six months prior, he was initiated on tirzepatide 2.5 mg weekly for obesity, with a plan to increase the dose every 4 weeks as tolerated and follow up in a month. At that time, his thyrotropin level was 1.9 milli-international units/L, and he was receiving the same dose of levothyroxine (200 mcg daily). He missed follow up visits after tirzepatide initiation due to living seasonally in different locations, however the tirzepatide dose was increased every 4 weeks up to the current dose of 10 mg weekly. Atrial fibrillation was determined to be due to thyrotoxicosis from excessive levothyroxine exposure 2.
