How we grade severity & evidence
Severity levels
- Contraindicated: These should generally not be used together.
- Major: Potentially serious — often needs a change or close monitoring.
- Moderate: Can be significant — usually manageable with monitoring.
- Minor: Usually limited clinical impact.
Evidence grades
- Established: Well documented — supported by controlled studies or strong clinical data.
- Probable: Good supporting evidence, though not definitively proven.
- Suspected: Some evidence suggests this interaction, but it is not well established.
- Possible: Limited or conflicting evidence; the interaction may occur.
- Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.
Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.
What happens
Reduced corticosteroid exposure
Interaction Deep Dive
Concurrent administration of phenytoin or fosphenytoin (prodrug of phenytoin) and corticosteroids have resulted in reduced corticosteroids exposure. Consider corticosteroids dose adjustments to achieve optimal clinical response when phenytoin or fosphenytoin is added to or withdrawn from a patient's regimen123.
Why it happens (mechanism)
Induction of hepatic drug-metabolizing enzymes by phenytoin
Literature reports
3 reports — tap to read
a) Concomitant methylPREDNISolone and phenytoin therapy has been reported to enhance the metabolism of the corticosteroid probably as a result of microsomal liver enzyme induction by phenytoin. One study stated the baseline half-life was decreased by 50% in patients on concurrent therapy 4.
b) Phenytoin has been demonstrated to increase the hepatic metabolism of corticosteroids. Those steroids with a pronounced effect include dexAMETHasone, fludrocortisone, methylPREDNISolone, and predniSONE 567.
c) Phenytoin has also been reported to reduce the therapeutic effects of prednisoLONE 8.9.
