For patients whose QTc exceeds 500 ms: hold vepdegestrant until the QTc is 480 ms or lower and less than 60 ms above baseline. Should a reversible cause be found and corrected, resume vepdegestrant at the same dose. Should no reversible cause be found, resume vepdegestrant at a lowered dose. Monitor ECGs more frequently, and permanently stop vepdegestrant should QTc greater than 500 ms occur again. For patients who develop torsade de pointes, polymorphic ventricular tachycardia, or symptoms of serious arrhythmia: permanently discontinue vepdegestrant1.
