a) A retrospective cohort study (n=234) found a heightened risk of clinically significant bleeding (defined as hospital admission for bleeding) among patients taking warfarin for atrial fibrillation while concurrently on SSRI therapy or within 2 weeks after discontinuing SSRI therapy. Patients averaging 72 +/- 7 years of age who received warfarin plus an SSRI (n=117) were matched against randomly chosen patients treated with warfarin alone (n=117). The SSRIs used were FLUoxetine, citalopram, PARoxetine, sertraline, fluvoxaMINE, or escitalopram. Nine patients had 11 bleeding episodes across 213.9 treatment years in the warfarin plus SSRI group, while 10 patients had 14 bleedings across 586.4 treatment years in the warfarin-only group. The corresponding overall bleeding incidences were 51.4 and 23.9 per 1000 treatment years, respectively. The risk of first bleedings while on warfarin plus SSRI was significantly elevated by 3.5 times relative to warfarin alone. Among patients who bled during concomitant use, the SSRIs involved were sertraline or citalopram. Adding an SSRI did not correspond to any change in warfarin dose or INR. The study's findings may be constrained by prior bleeding events, uncertain patient adherence, and the omission of clopidogrel, dipyridamole, corticosteroids, and anticoagulants besides warfarin from the model 2.

c) An 83-year-old man died of cerebral hemorrhage resulting from elevated warfarin concentrations. He had been receiving warfarin 30 mg per week for atrial fibrillation with a target INR of 2 to 3. His other medications included lisinopril, furosemide, potassium, digoxin, and acetaminophen. After presenting with symptoms of depression, anxiety, and insomnia, he was prescribed FLUoxetine 20 mg per day and diazePAM 2.5 mg 3 to 4 times per day. Eight days afterward, his INR rose to 4.8, and the warfarin dose was reduced to 27.5 mg per week. He was subsequently seen for signs of drug delirium, including confusion, incoherence, and irrational speech. At that time, the INR was 3.64 and FLUoxetine was stopped. The following day he arrived at the hospital with left-sided weakness and later died from complications of a large interparenchymal hemorrhage. The authors suggested that adding FLUoxetine to his regimen led to increased serum levels of both warfarin and diazePAM, producing delirium and loss of anticoagulant control 5.
d) A population-based, case-controlled study of new coumarin users (acenocoumarol and phenprocoumon) taking concomitant selective serotonin reuptake inhibitors (SSRIs) found an elevated risk of hospitalization for non-gastrointestinal bleeding. Drawing on national pharmacy and hospitalization records in the Netherlands, researchers identified 1848 cases admitted for abnormal bleeding and compared them against 5818 control subjects who were also on a coumarin. The median treatment duration among patients was 220 days (range, 1 to 4690 days). Patients on SSRIs had a greater risk of hospitalization for non-gastrointestinal bleeding (adjusted odds ratio (OR) 1.7, 95% confidence interval (CI), 1.1 to 2.5); however, the rate of gastrointestinal bleeding (adjusted OR 0.8, 95% CI, 0.4 to 1.5) did not differ significantly 6.
