Phosphate binders (eg, sevelamer) may bind to levothyroxine preventing absorption and potentially resulting in hypothyroidism32. In a study, concomitant use of levothyroxine and sevelamer in healthy euthyroid individuals resulted in reduced T4 absorption by 49.6% 4. Avoid concomitant use if possible. If coadministration is required, maximally separate administration 1 and administer levothyroxine at least 4 hours apart from sevelamer 32.

a) In pharmacokinetics studies of levothyroxine conducted in healthy euthyroid volunteers (n=7; age range, 22 to 40 years), levothyroxine absorption was decreased (ie, a smaller serum thyroxine (T4) absorption AUC) following the concurrent use of levothyroxine and sevelamer compared with the use of levothyroxine alone. In four 6-hour studies performed at least 2 weeks apart, participants who had fasted overnight received levothyroxine 1 mg (five 0.2-mg tablets) orally alone in the first study and levothyroxine 1 mg with ezetimibe 10 mg, sevelamer hydrochloride 800 mg and chromium picolinate 1000 mcg, respectively, in the 3 subsequent studies. Blood samples were collected 15 minutes before, at the time of, and 30, 60, 120, 240, and 360 minutes after administration in each study. The area between baseline and the T4 absorption curve was 2176 +/- 195 mcg-minutes/dL for levothyroxine administered alone compared with 1178 +/- 281 mcg-minute/dL for levothyroxine administered with sevelamer (50.4% of T4 alone). The mean baseline serum of TSH level was 2.81 +/- 0.59 microunits/mL for levothyroxine alone and 2.11 +/- 0.39 microunits/mL for levothyroxine plus sevelamer; none of the participants showed any symptoms of thyrotoxicosis. Although investigators postulated that the mechanism of this interaction between levothyroxine and sevelamer was the adsorption of thyroxine by sevelamer, other possible mechanisms (eg, via drug-induced alterations in mucosal transport processes) could not be ruled out 4.
