Sodium Bicarbonate
Cefpodoxime
How we grade severity & evidence
Severity levels
- Contraindicated: These should generally not be used together.
- Major: Potentially serious — often needs a change or close monitoring.
- Moderate: Can be significant — usually manageable with monitoring.
- Minor: Usually limited clinical impact.
Evidence grades
- Established: Well documented — supported by controlled studies or strong clinical data.
- Probable: Good supporting evidence, though not definitively proven.
- Suspected: Some evidence suggests this interaction, but it is not well established.
- Possible: Limited or conflicting evidence; the interaction may occur.
- Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.
Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.
What happens
Decreased cefpodoxime effectiveness
Interaction Deep Dive
Absorption of cefpodoxime may be impaired if aluminum, calcium, or magnesium containing products are coadministered. Drugs that raise gastric pH reduce peak cefpodoxime concentrations. Administration of cefpodoxime with either aluminum hydroxide (120 mL) or sodium bicarbonate (12.6 g) significantly reduced the peak concentration and AUC compared to administration during fasting or with food1. A 40% decrease in the AUC of cefpodoxime has been reported when administered after two doses of an aluminum-magnesium antacid 2.
Why it happens (mechanism)
Decreased cefpodoxime absorption

a) Concomitant administration of high doses of antacids (sodium bicarbonate and aluminum hydroxide) and cefpodoxime reduces peak plasma levels of cefpodoxime by 24% to 42% and the extent of absorption by 27% to 32% 3.
