Pilocarpine falls under Topical Drugs — the group Alchemilla interacts with.

While no side effects have been reported from orally using Alchemilla, a clinical study titled "Treatment of Aphthous Stomatitis with Topical Alchemilla vulgaris in Glycerine," published in Clinical Drug Investigation (https://pubmed.ncbi.nlm.nih.gov/17163290/), found that local mouth irritation occurred after applying a commercial product containing 3% alchemilla extract and glycerol (Aphtarine, Laboratoires Biosphere). Aphtarine was non-irritating in 68.2% of cases within 2-3 days, in 18.8% of cases within 4-7 days, and in 13.6% of cases with moderate irritation. Theoretically, combining Alchemilla with other potentially irritating topical products could increase the risk of local irritation and discomfort. The drug in this drug interaction monograph is available in a topical dosage form. There is no interaction expected from the oral dosage form of the drug.

Good news first: this isn't a deep chemical interaction. Alchemilla is loaded with tannins, which have an astringent, drying, and sometimes irritating effect on tissue. The concern here is purely local. If you use a topical Alchemilla product (like a mouth gel or rinse) in the same area where you apply topical pilocarpine, the two together could add up to more mouth or gum irritation, redness, or a stinging feeling. A small study using an oral Alchemilla product did report some mild mouth irritation, so the worry isn't imaginary, just minor.

Important: if you take pilocarpine as a tablet (Salagen) or eye drops (Vuity), there's really no expected interaction with Alchemilla at all. Either way, run it by your pharmacist before combining them so we can keep your mouth comfortable.

Alchemilla (lady's mantle) is a European herb traditionally used for digestive complaints, women's health issues, and as an astringent for skin and wounds. Solid human research is very limited, so its benefits remain largely unproven, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

About Pilocarpine

What Is Pilocarpine Used For?

Brand names: Salagen, Pilocar, Vuity, Qlosi

Pilocarpine eye drops are used to lower elevated pressure inside the eye in people with open-angle glaucoma or ocular hypertension, to manage acute angle-closure glaucoma, to prevent pressure spikes after laser eye surgery, and to cause pupil constriction (miosis) when needed. Vuity and Qlosi eye drops, as well as Pilocarpine Hydrochloride Ophthalmic solution, are specifically approved to treat presbyopia (age-related difficulty focusing up close) in adults.

Pilocarpine tablets — including Salagen and generic pilocarpine hydrochloride tablets — are used by mouth to relieve dry mouth caused by salivary gland damage from radiation therapy for head and neck cancer, or from Sjögren's syndrome (an autoimmune condition that reduces saliva and tear production).
