Ivermectin falls under Topical Drugs — the group Alchemilla interacts with.

While no side effects have been reported from orally using Alchemilla, a clinical study titled "Treatment of Aphthous Stomatitis with Topical Alchemilla vulgaris in Glycerine," published in Clinical Drug Investigation (https://pubmed.ncbi.nlm.nih.gov/17163290/), found that local mouth irritation occurred after applying a commercial product containing 3% alchemilla extract and glycerol (Aphtarine, Laboratoires Biosphere). Aphtarine was non-irritating in 68.2% of cases within 2-3 days, in 18.8% of cases within 4-7 days, and in 13.6% of cases with moderate irritation. Theoretically, combining Alchemilla with other potentially irritating topical products could increase the risk of local irritation and discomfort. The drug in this drug interaction monograph is available in a topical dosage form. There is no interaction expected from the oral dosage form of the drug.

Here's the simple version: this isn't about ivermectin getting into your blood or being broken down differently. It only applies to the skin or topical forms of ivermectin, like Soolantra cream or Sklice lotion. Alchemilla is loaded with tannins, which are astringent and can dry out and irritate tissue. A small study even found mild mouth irritation when a topical Alchemilla product was used.

So the worry is purely local skin irritation: putting Alchemilla on the same patch of skin where you apply ivermectin could add up to more redness, stinging, or dryness. This is a minor, theoretical concern. If you take ivermectin as a pill, there's no expected interaction at all. If you use the cream, just check with your pharmacist before combining them on your skin.

Alchemilla (lady's mantle) is a European herb traditionally used for digestive complaints, women's health issues, and as an astringent for skin and wounds. Solid human research is very limited, so its benefits remain largely unproven, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

About Ivermectin

What Is Ivermectin Used For?

Brand names: Stromectol, Mectizan, Sklice, Soolantra

Ivermectin tablets (including Stromectol) are used to treat two parasitic infections: strongyloidiasis (a roundworm infection of the intestinal tract) and onchocerciasis (river blindness, caused by a parasitic worm spread by blackfly bites). These are prescription uses for serious infections.

Topical ivermectin lotion — sold under several names including RID One and Done, Head Lice Treatment Ivermectin Lotion, Scalp Care Lotions, and others — is applied to the scalp to treat head lice. A separate topical ivermectin cream (1%), available as Soolantra and ivermectin cream 1%, is used to treat the inflammatory pimple-like bumps of rosacea.
