Theoretically, anise oil might increase the effects and adverse effects of codeine.
Animal research shows that anise oil increases the analgesic effects of codeine, possibly by inducing its phase I metabolism and increasing conversion to morphine. Whether this interaction occurs in humans is unclear.

Here's something worth knowing: codeine is what we call a prodrug. On its own it doesn't do much. Your liver has to convert it into morphine to get the real pain relief. So anything that speeds up that conversion can actually make codeine feel stronger.

That's what's going on here. In animal studies, anise oil appears to ramp up the enzyme that turns codeine into morphine, which boosted the pain-relieving effect. In theory, more morphine on board could also mean more of codeine's usual downsides, like drowsiness, constipation, nausea, or slowed breathing. I want to be honest though: this is only lab and animal data, so we don't truly know if it matters in people. A little anise in cooking or a cup of anise tea is very unlikely to be an issue. If you're using concentrated anise oil regularly, just mention it to your pharmacist or doctor.
