NEPHROTOXIC DRUGS
Why this applies to Acetaminophen, Phenylephrine, Chlorpheniramine
Acetaminophen, Phenylephrine, Chlorpheniramine falls under NEPHROTOXIC DRUGS — the group Aristolochia interacts with.
Some potentially nephrotoxic drugs include cyclosporine (Neoral, Sandimmune); aminoglycosides including amikacin (Amikin), gentamicin (Garamycin, Gentak, others), and tobramycin (Nebcin, others); nonsteroidal anti-inflammatory drugs (NSAIDs) including ibuprofen (Advil, Motrin, Nuprin, others), indomethacin (Indocin), naproxen (Aleve, Anaprox, Naprelan, Naprosyn), piroxicam (Feldene); and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Let me be straight with you: the bigger problem here is Aristolochia itself. It contains aristolochic acid, which is a known kidney toxin and is linked to permanent kidney damage and cancer. The FDA has warned against any product containing it, so I would not recommend taking it at all, regardless of what else you're on.
As for your cold tablets (acetaminophen, phenylephrine, and chlorpheniramine), the concern is mostly with the acetaminophen (Tylenol) piece. At high doses or over long stretches, acetaminophen can stress the kidneys and liver. Pairing it with something already hard on the kidneys could, in theory, add up. To be clear, this combined effect is theoretical and hasn't actually been reported in people. Still, please talk with your pharmacist or doctor before using Aristolochia.
