Why this applies to Clozapine

Clozapine falls under ANTICHOLINERGIC DRUGS — the group Betel Nut interacts with.

In vitro and animal research suggests that betel nut extract can inhibit acetylcholinesterase activity. Additionally, in one case series, adults who were treated with procyclidine, an anticholinergic drug, and neuroleptics for the management of extrapyramidal symptoms (EPS) due to fluphenazine ultimately developed severe EPS, such as tremor and stiffness, thought to be related to betel nut use. Theoretically, concurrent use of anticholinergic drugs and betel nut might decrease the effectiveness of betel nut or the anticholinergic agent.
Some anticholinergic drugs include atropine, benztropine (Cogentin), biperiden (Akineton), procyclidine (Kemadrin), and trihexyphenidyl (Artane). Avoid concomitant use.

Here's the gist for clozapine: part of how this medicine works (and part of why it causes side effects) is by blocking acetylcholine, a nerve-signaling chemical. Betel nut seems to push in the opposite direction. Lab and animal studies suggest its main compound, arecoline, boosts acetylcholine activity, which could partly cancel out clozapine's anticholinergic effects. In one older case series, betel nut use seemed to worsen muscle stiffness and tremor in people on antipsychotic-type medicines.

This is theoretical, based mostly on pharmacology and a few case reports, not strong human studies. Still, with clozapine I'd steer clear of betel nut entirely, since the nut itself is linked to mouth cancer and addiction. If you chew it, please mention it to your prescriber or pharmacist before making any changes.

What Is Clozapine Used For?

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
