Why this applies to Hydroxyzine

Hydroxyzine falls under ANTICHOLINERGIC DRUGS — the group Betel Nut interacts with.

In vitro and animal research suggests that betel nut extract can inhibit acetylcholinesterase activity. Additionally, in one case series, adults who were treated with procyclidine, an anticholinergic drug, and neuroleptics for the management of extrapyramidal symptoms (EPS) due to fluphenazine ultimately developed severe EPS, such as tremor and stiffness, thought to be related to betel nut use. Theoretically, concurrent use of anticholinergic drugs and betel nut might decrease the effectiveness of betel nut or the anticholinergic agent.
Some anticholinergic drugs include atropine, benztropine (Cogentin), biperiden (Akineton), procyclidine (Kemadrin), and trihexyphenidyl (Artane). Avoid concomitant use.

Here is the simple version. Hydroxyzine is an antihistamine that also has mild anticholinergic activity, meaning it tends to quiet certain nerve signals. Betel nut works in the opposite direction. Its main compound, arecoline, acts a bit like a nerve stimulant and can block an enzyme (acetylcholinesterase), which effectively boosts those same nerve signals. So the two can work against each other, and you may notice your hydroxyzine not calming itching or anxiety as well as usual.

Keep in mind this concern is mostly theoretical, based on lab and animal research plus a few case reports, so the real-world effect in any one person is uncertain. That said, betel nut carries serious health risks of its own and is best avoided entirely. Please check with your pharmacist or doctor before mixing the two.

About Hydroxyzine

What Is Hydroxyzine Used For?

Brand names: Atarax, Vistaril, Ucerax

Hydroxyzine is used to relieve anxiety and tension related to psychoneurosis (anxiety-based mental health conditions) and as a supportive treatment when anxiety is part of another medical condition. Oral forms — including tablets, capsules, and oral solution — are used for anxiety relief and to manage itching (pruritus) caused by allergic conditions such as chronic urticaria (hives), atopic dermatitis, contact dermatitis, and histamine-driven itch. Oral forms are also used as a sedative before and after surgery or general anesthesia.

Hydroxyzine Hydrochloride intramuscular injection is used specifically when a shot is needed — for example, in acute psychiatric or emotional emergencies, acute alcohol withdrawal with anxiety symptoms or delirium tremens, control of nausea and vomiting (except during pregnancy), and as pre- and post-operative or pre- and post-partum supportive medication to reduce narcotic requirements and control nausea.
