Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cranberry interacts with.

Theoretically, cranberry might increase the levels and adverse effects of CYP3A4 substrates.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.

Here's the idea behind this pairing. Your body relies on a liver enzyme called CYP3A4 to break down acalabrutinib. Cranberry, in concentrated amounts, may slow that enzyme down a little. If that happens, acalabrutinib could clear more slowly and build up, which in theory could mean stronger effects and a higher chance of side effects like bruising, bleeding, headaches, infections, or low blood counts.

I want to be honest about the evidence here, though. This is mostly theoretical, drawn from a single case report with a different drug and some animal research. A glass of cranberry juice or a handful of berries is unlikely to cause trouble. Larger daily amounts or concentrated supplements are where I'd be more cautious. Since acalabrutinib is a serious cancer medicine, please run any regular cranberry use by your oncology team or pharmacist first.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
