Why this applies to Ezetimibe, Atorvastatin

Ezetimibe, Atorvastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cranberry interacts with.

Theoretically, cranberry might increase the levels and adverse effects of CYP3A4 substrates.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.

Your Liptruzet combines two cholesterol medicines, and one of them, atorvastatin, leans on a liver enzyme called CYP3A4 to be cleared from your body. The idea here is that cranberry might slow that enzyme down a little, which could let atorvastatin build up higher than usual. When statin levels run high, the main worry is muscle problems (aches, weakness, or in rare cases a serious breakdown called rhabdomyolysis) and occasionally stress on the liver.

That said, this is built on very thin evidence, basically a single case report involving a lot of cranberry juice (16 ounces a day) plus some animal data. A glass now and then or a normal cranberry supplement is unlikely to be a problem for most people. Still, if you notice new muscle soreness, dark urine, or unusual tiredness, check in with your pharmacist or doctor before changing anything.

Theoretically, cranberry might increase levels and adverse effects of atorvastatin.
In one case report, a patient taking atorvastatin experienced upper back pain, rhabdomyolysis, and abnormal liver function after drinking cranberry juice 16 ounces daily for 2 weeks. Theoretically, this may have been caused by inhibition of cytochrome P450 3A4 (CYP3A4) enzymes by cranberry juice, as atorvastatin is a CYP3A4 substrate. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Patients taking atorvastatin should avoid large quantities of cranberry juice.

Your combo pill (Liptruzet) pairs ezetimibe with atorvastatin, and it's the atorvastatin half we're watching here. Atorvastatin is cleared by a liver enzyme called CYP3A4. There's a theory that cranberry can slow that enzyme down a bit, which could let atorvastatin build up in your system and raise the odds of statin side effects like muscle aches, weakness, or, rarely, liver irritation.

Honestly, this is built on a single case report where someone drank about 16 ounces of cranberry juice daily for two weeks, so the evidence is thin. An occasional glass or normal food amounts are unlikely to cause trouble. If you'd be having large daily amounts or concentrated supplements, just run it by your pharmacist or doctor first. And let them know right away if you notice new, unexplained muscle pain.
