CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Macitentan, Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cranberry interacts with.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi combines two active medicines, macitentan and tadalafil, and both are cleared by a liver enzyme called CYP3A4. In theory, cranberry may slow that enzyme a little, which could let these drugs build up somewhat. If levels rise, you might notice more of their usual effects, like headache, flushing, stuffy nose, low blood pressure, dizziness, or swelling in the legs.
Honestly though, the evidence here is thin. It comes from one case report with a different drug and some animal data, so it is more of a theoretical concern than a proven problem. Occasional cranberry, or a normal glass of juice, is unlikely to cause trouble. If you plan to use concentrated cranberry supplements daily, just run it by your pharmacist or doctor first so they can keep an eye on things.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
2 of 2Macitentan, Tadalafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
There is contradictory evidence about the effect of cranberry on CYP2C9 enzymes. In vitro evidence suggests that flavonoids in cranberry inhibit CYP2C9 enzymes. However, clinical research shows that cranberry juice does not significantly affect the levels, metabolism, or elimination of the CYP2C9 substrates flurbiprofen or diclofenac. Also, in patients stabilized on warfarin, drinking cranberry juice 250 mL daily for 7 days does not significantly increase the anticoagulant activity of warfarin, a CYP2C9 substrate. Additional pharmacokinetic research shows that cranberry juice does not increase peak plasma concentrations or area under the concentration-time curve of warfarin.

Good question about pairing cranberry with Opsynvi (the macitentan and tadalafil combo). The idea here is that plant compounds in cranberry might slow down a liver enzyme called CYP2C9 that helps clear these medicines. If that happened, drug levels could creep up a bit, and you might notice more of Opsynvi's usual effects like low blood pressure, dizziness, flushing, headache, or leg swelling.
Here's the reassuring part: this is mostly a theory from lab dishes, not real people. When researchers actually tested cranberry against similar drugs, they found little to no meaningful change. Everyday cranberry juice or a supplement is unlikely to cause trouble. Still, since Opsynvi affects blood pressure, mention it to your pharmacist or doctor if you use cranberry regularly.
