Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cranberry interacts with.

Theoretically, cranberry might increase the levels and adverse effects of CYP3A4 substrates.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.

Here's the basic idea: your body uses an enzyme called CYP3A4 to help clear voxelotor (Oxbryta) out of your system. Cranberry, in concentrated amounts, may gently slow that enzyme down. If that happens, voxelotor could build up a little more than expected, which in theory might mean stronger effects or a higher chance of side effects like headache, nausea, diarrhea, or stomach discomfort.

I want to be honest about how thin this evidence is. The concern comes from a single case report with a different drug and some animal data, not from studies on voxelotor itself. An occasional glass of cranberry juice or a few berries almost certainly won't be a problem. Still, before you start a daily cranberry supplement, run it by your pharmacist or doctor.

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.

This approval was granted under the FDA's accelerated approval pathway, based on the drug's ability to raise hemoglobin levels. Continued approval may depend on results from confirmatory clinical trials showing broader clinical benefit.
