Here's the gist for anyone using cannabis or a THC product while on Kisqali (ribociclib). Your body clears THC partly through a liver enzyme called CYP3A4, and ribociclib happens to slow that same enzyme down. So in theory, ribociclib could let THC build up a bit more than usual, meaning a stronger or longer-lasting high, more drowsiness, dizziness, dry mouth, or feeling 'out of it.'

I want to be honest about the evidence here: this is a theoretical concern based on how the two work, not something proven in studies. Ribociclib is not a prodrug, so it doesn't flip the direction. Still, since both can cause tiredness and affect your thinking, it's worth a quick chat with your pharmacist or oncologist before combining them. Please don't change anything on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Ribociclib Succinate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Your breast cancer medicine, ribociclib (Kisqali), is cleared from the body by a liver enzyme called CYP3A4. Some lab tests suggest THC might slightly slow that enzyme down. If that happened, ribociclib could build up a bit and, in theory, push up the risk of its known side effects like nausea, low white blood cell counts, fatigue, or changes in your heart's QT rhythm.

The good news is this concern is very weak. It comes from test-tube studies, and a study in healthy people found that oral THC did not actually block CYP3A4. So for most people this is unlikely to cause a real problem. Still, since ribociclib needs careful monitoring, please mention any cannabis or THC use to your oncologist or pharmacist so they can keep an eye on things.

What Is Ribociclib Used For?

Brand name: Kisqali

Kisqali is used to treat adults with hormone receptor (HR)-positive, HER2-negative breast cancer. For early breast cancer (stage II or III, at high risk of recurrence), Kisqali is used together with an aromatase inhibitor as adjuvant treatment — meaning it's given after initial treatment to help reduce the chance the cancer comes back.

For advanced or metastatic breast cancer (cancer that has spread), Kisqali is used in combination with either an aromatase inhibitor or fulvestrant — either as the first hormone-based treatment or after the cancer has progressed on another hormone therapy.
