Why this applies to Darunavir, Cobicistat

Darunavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the gist for your Prezcobix (darunavir with cobicistat). Your body clears darunavir mainly through a liver enzyme called CYP3A4, and the cobicistat in this combo is actually there to slow that enzyme down on purpose, keeping darunavir levels high enough to work. In lab studies, Fo-ti may also nudge that same enzyme in the slowing direction, which could in theory push darunavir levels up a bit further.

If that happened, you might notice more of darunavir's usual side effects, like nausea, diarrhea, headache, or a rash. Honestly, though, this is based only on test-tube data and hasn't been seen in real people, and the results even conflict in animal studies. Since Fo-ti also carries its own liver-injury concerns, I'd run it by your HIV pharmacist or doctor before adding it.

What Is Darunavir and Cobicistat Used For?

Brand name: Prezcobix

Prezcobix and Prezcobix Ped are used to treat HIV-1 (human immunodeficiency virus type 1) infection. They are always used together with other antiretroviral (anti-HIV) medicines — never alone. Both products are approved for people who are new to HIV treatment (treatment-naïve) and for those who have taken HIV medicines before (treatment-experienced), as long as certain darunavir resistance mutations are not present.

Prezcobix tablets are approved for adults and pediatric patients weighing at least 40 kg. Prezcobix Ped tablets for oral suspension extend coverage to pediatric patients 3 years of age and older weighing at least 15 kg.
